This section explains your rights regarding personal information and how requests for information held by the practice may be made.

Our Commitment to POPIA

The practice aims to collect and process personal information lawfully, fairly and responsibly in accordance with the Protection of Personal Information Act 4 of 2013 (POPIA). Personal information is collected for clear purposes, limited to what is reasonably needed and protected through appropriate safeguards.

Your Rights

Subject to applicable law, you may:

  • Ask whether the practice holds personal information about you.
  • Request access to your personal information.
  • Ask for incorrect or incomplete information to be corrected.
  • Object to certain uses of your information where the law allows.
  • Withdraw consent where consent is the basis for processing.
  • Raise a concern about how your information is handled.

Making a Request

Requests should be made in writing and should explain what information or record you are requesting. You may be asked to verify your identity before information is provided. Access may be limited where the law protects confidential information, another person's privacy or other relevant interests.

Access to Information under PAIA

Requests for access to records will be considered in accordance with the Promotion of Access to Information Act 2 of 2000 (PAIA), as amended, and other applicable laws. Formal requests may require prescribed forms or fees. Information about the practice's PAIA procedures should be made available where required by law.

Security Concerns

If a personal information security incident occurs, the practice will take appropriate steps and notify the relevant parties or authorities where required by law.
